PCSK9 inhibitors such as evolocumab (Repatha) and alirocumab (Praluent) are injectable medications that lower LDL cholesterol substantially, typically for patients at high cardiovascular risk or with very high cholesterol. Doctors On Site connects Ontario patients with licensed physicians who assess eligibility and, where clinically appropriate, prescribe or coordinate specialist care.

They are generally reserved for people who need significant additional LDL lowering — for example, those with familial hypercholesterolemia, established cardiovascular disease that is not controlled on other therapy, or statin intolerance. They are available in Ontario by prescription, and access often involves specific coverage criteria.

PCSK9 inhibitors block a protein (PCSK9) that reduces the liver's ability to clear LDL cholesterol from the blood. By blocking it, the liver removes more LDL, lowering blood cholesterol significantly.
