PCSK9 Inhibitors in Ontario — Physician Assessment

PCSK9 inhibitors such as evolocumab (Repatha) and alirocumab (Praluent) are injectable medications that lower LDL cholesterol substantially, typically for patients at high cardiovascular risk or with very high cholesterol. Doctors On Site connects Ontario patients with licensed physicians who assess eligibility and, where clinically appropriate, prescribe or coordinate specialist care.

What are PCSK9 inhibitors?

PCSK9 inhibitors are a newer class of cholesterol-lowering medications given by injection (usually every two or four weeks). They are generally reserved for people who need significant additional LDL lowering — for example, those with familial hypercholesterolemia, established cardiovascular disease that is not controlled on other therapy, or statin intolerance. They are available in Ontario by prescription, and access often involves specific coverage criteria.

Who commonly uses PCSK9 inhibitors?

Doctors On Site physicians commonly assess patients seen through our programs, including:

Familial hypercholesterolemia Very high LDL despite statin and ezetimibe therapy Established cardiovascular disease needing further LDL lowering Statin intolerance with high cardiovascular risk

How do PCSK9 inhibitors work?

PCSK9 inhibitors block a protein (PCSK9) that reduces the liver's ability to clear LDL cholesterol from the blood. By blocking it, the liver removes more LDL, lowering blood cholesterol significantly. Because of how they are used and funded, a physician carefully assesses whether they are appropriate and coordinates specialist input where needed.

Who may qualify?

  • Patients with high cardiovascular risk needing major LDL reduction
  • Those who meet clinical criteria after a physician assessment
  • Patients without contraindications identified during the assessment
  • Those who can attend follow-up and any required specialist review
